Empty sella

Whenever you see an empty Sella you should think about two differential diagnosis:
The first one is empty Sella syndrome while the second one is congenital absence of the pituitary gland, but let us discuss how we can diagnose empty Sella.
Empty Sella syndrome means that the junction between the pituitary stalk and upper surface of the pituitary gland is situated below the level of the posterior clinoids ( normally it should be situated at the level of posterior clinoid), while congenital absence of the pituitary gland is diagnosed by complete absence of the pituitary gland and its stalk.
